As a father or father figure in the life of a child(ren), your thoughts and experiences are important to us. The Central and North Central Florida Coalitions are conducting a community needs assessment and your input will help us better understand local needs to determine how we can improve services, health outcomes, and the well-being of individuals and families in your (and our) community.
This survey should take no more than 10 minutes to complete. It's anonymous, your responses will be kept confidential, and the information you provide will not be used to identify you. To be eligible, you must be 18 years or older, be a father or father figure to a child aged 0 (not yet born) through 5 years of age, and live within our 16-County service area (Alachua, Bradford, Citrus, Columbia, Dixie, Gilchrist, Hamilton, Hernando, Lafayette, Lake, Levy, Marion, Putnam, Sumter, Suwannee and Union counties).
