UBC-Empowering the Next Generation Program Participant Application Form

1.By completing and submitting this application, I confirm that the information provided is accurate and truthful. I acknowledge that the Empowering the Next Generation Program (ENGP) is federally funded and is in partnership with the United Brotherhood of Carpenters & Joiners of America and that any financial support provided for in-class training and on-the-job training is contingent upon my continued participation in both components.

If my application is approved, I understand that I must immediately notify ENGP@ubcja.ca should I voluntarily withdraw from, or be removed from, either the in-class or on-the-job training. I further acknowledge that repayment of any funds deposited to me in advance may be required, and I agree to repay such funds if and when notified.(Required.)

6.This form is designed to collect information for the Empowering the Next Generation Program. Your response is voluntary, and you may identify in more than one designated group.

The information you provide will be used to provide project data to the Government of Canada (GoC) for the purpose of monitoring the ENGP, assessing its contribution to the overall objectives of the funding program, and to inform future policy development.

All data collected is strictly confidential, and the information provided to the Government of Canada (GoC) will be anonymous and confidential.

7.A person with a disability
