Participant Disclosure Information

The 40th AACVPR Annual Meeting will be jointly accredited by Creighton University Health Sciences Continuing Education. Accredited as a Jointly Accredited Provider by the Accreditation Council for Continuing Medical Education (ACCME) and the American Nurses Credentialing Center (ANCC). Creighton University Health Sciences Continuing Education must ensure balance, independence, objectivity, and scientific rigor in all individually provided or jointly provided educational activities.

Educational content must be free of commercial influence. Individuals who are in a position to control content of this educational activity are expected to disclose all financial relationships with ineligible companies that may pose a real or apparent conflict(s) of interest that may have a direct bearing on the subject matter of the continuing education activity.

ACCME defines an ineligible company as: “any entity producing, marketing, selling, re-selling, or distributing health care products used by or on patients”. Examples of such organizations include:

  • Advertising, marketing, or communication firms whose clients are ineligible companies
  • Bio-medical startups that have begun a governmental regulatory approval process
  • Compounding pharmacies that manufacture proprietary compounds
  • Device manufacturers or distributors
  • Diagnostic labs that sell proprietary products
  • Growers, distributors, manufacturers or sellers of medical foods and dietary supplements
  • Manufacturers of health-related wearable products
  • Pharmaceutical companies or distributors
  • Pharmacy benefit managers
  • Reagent manufacturers or sellers

Individuals must disclose regardless of their view of the relevance of the relationship to education. Examples of financial relationships include:
  • Employee
  • Researcher
  • Consultant
  • Advisor
  • Speaker
  • Independent contractor (including contracted research)
  • Royalties or patent beneficiary
  • Executive role
  • Ownership interest

The ACCME does not consider providers of clinical service directly to patients as ineligible companies. An ineligible company is not eligible for ACCME accreditation. Please carefully review the above Creighton University Policy on Conflict of Interest and complete this form to indicate any relationships. Important: all relationships within the proceeding 24-months must be disclosed. Your completion of this form is your Declaration of Disclosure.
