Public Service Loan Forgiveness Survey 

Thank you for taking this survey! It should take no more than 10 minutes complete. We will be asking questions about your experience applying and being denied Public Service Loan Forgiveness.
1.Do you have federal student loans?
2.When did you start repaying your student debt?
3.Has there been a break in your repayment since then (e.g., due to going back to school, financial difficulties, deferments, etc)
4.Have you ever consolidated your federal student loans (i.e. combined multiple loans into one loan or changed the type of loan you have)?
5.Which is your current student loan servicer? Check all that apply.
6.Have you filled out the final application for Public Service Loan Forgiveness (shown here)?
7.Did you receive written notification(similar to the letter below) from your servicer that you were denied PSLF? 
8.What reason were you given for denial of Public Service Loan Forgiveness?
9.After you were denied PSLF, did you apply for Temporary Expanded Public Service Loan Forgiveness by emailing the Dept. of Education?
10.Did you receive written notification from the Department of Education providing a reason for denying your TEPSLF application?

As part of our student debt efforts, we are trying to understand the broad impact on public service workers that the denial of PSLF has had across the country.
