BASE Parent Survey 2015-2016

Each year the BASE program at school is offered because of a grant that the district secured through the state and federal government. As part of this grant we must conduct end of the year survey's with students, staff, parents, and BASE workers. Students took their surveys the last night of BASE. I am sending this survey out to you because your child participated in BASE. You may take this survey with pencil and paper and return to school, or you can go to the district website and click under parent and there is a link to the survey on-line.
