Criteria for the Award: This award is designed to recognize CNA’s and NA’s in the home and hospice care industry. The nominee must have a minimum of one (1) year experience working in home care and/or hospice.

They must demonstrate the following:
• A commitment to the industry by serving on internal agency committees, AHHC committees or community involvement;
• An interest in professional growth through continuing education;
• Excellence and commitment to patient care; and
• Dedication to raising the level of professionalism.

Do not use the name of the nominee or any identifying information (such as agency name) on your narrative or in your answers.
