Thank you for your participation in Design for Change! This year we are conducting a national impact analysis of our program and are asking all DFC mentors to complete a short survey before and after implementing DFC. You responses will be carefully considered as we work to determine the current impact of DFC and how we can make DFC even better in the future. We rely on project mentors as the leading experts of their context and best reporters of the  experience DFC creates for students. Please respond to the following questions as carefully and thoroughly as you are able, your contribution to this study is of immense value!

Because we know your time is valuable, we will enter all DFC mentors who complete both the pre-survey and the post-survey in full into a drawing to win one of six $50 Amazon gift cards.

Before proceeding with the survey, please read the following informed consent statement.

Consent for Participation in Research

I volunteer to participate in a research project done by researchers at Design for Change USA.  I understand that the project is designed to gather information about the mentor and student experience in Design for Change.

1. My participation in this project is voluntary. I understand that I will not be paid for my participation. I may stop participation at any time without penalty. If I decline to participate or withdraw from the study, that information will be kept confidential.

2. I understand that while risk to participation in this study is considered minimal, there may be some risk in sharing personal information on the survey. If, however, I feel uncomfortable in any way during the survey, I have the right to decline to answer any question or to end the survey.

3. Participation involves completing a survey about 20-30 minutes in length with both an answer selection section and free response section.

4. I understand that the researcher will not identify me by name in any reports using information obtained from the surveys, and that my confidentiality as a participant in this study will remain secure. Subsequent uses of records and data will be subject to standard data use policies, which protect the anonymity of individuals and institutions. All files will be kept digitally on a secure, password protected hard drive.

5. Faculty and administrators from my campus will have no access to raw data collected in this study. This precaution will prevent my individual comments from having any negative repercussions. Faculty and administration may have access to the final research report.

6. I have read and understand the explanation provided to me. I have had all my questions answered to my satisfaction, and I voluntarily agree to participate in this study.
