Access to Food Survey

This survey is conducted on behalf of the Centre Wellington Community Foundation and the Rural Wellington health care partners. The purpose is to better understand food insecurity and what the barriers are to accessing sufficient, affordable, nutritious food. Once the information is gathered, the results will be made available and will inform what can be done to improve food security.

The survey is confidential (if you add your email address) or anonymous if you do not. You may quit the survey at any time. Questions about the survey can be sent to karmstrong@mountforestfht.com or 519-323-8389.
