Learn to Play: Ukulele Edition

The Learn To Play: Ukulele will have 3, 2-hour sessions: 
January 20, February 3, and February 10 from 11:00 am-1:00 pm
This free program is designed to introduce participants 14 and older to playing the Ukulele

Notes on the Program
This program is reserved for participants 14 and older. 
2 Hour Classes will be held January 20, February 3, and February 10 from 11:00 am-1:00 pm.
There will be a 30 minute meal break. The Lyric is not responsible for preparing foods appropriate to individual diets or restrictions, and neither The Lyric nor its employees, volunteers, or any associated staff responsible if a participant should consume restricted foods. 

Obligation Clause:
To provide the best and most well rounded educational and learning experience, I agree, to the best of my abilities, to attend all 3 Learn to Play Ukulele sessions with my provided instrument. I understand if I do not complete the program I must return the instrument provided. If I fail to do so I will be charged the full cost of the instrument. If I am unable to attend, I will notify The Lyric within 24 hours of the session’s occurrence that I will be absent. If I fail to notify The Lyric of my absence in advance, I understand that The Lyric may take action to the extent of removing myself from the program. I understand that, should I realize I will not be able to attend the Learn to Play Ukulele program, that I will notify The Lyric so another participant on the wait list for the course will be able to take part in the program.

Additional Important Information:
The Lyric Theatre & Cultural Arts Center is committed to conducting its programs and activities in the safest manner possible and holds the safety of participants in the highest possible regard. Participants must recognize there is an inherent risk of injury when choosing to participate. The Lyric Theatre strives to reduce such risk and insist that all participants follow safety rules and instructions which have been designed to protect the participant’s safety. Please recognize that The Lyric Theatre does not carry medical accident insurance for injuries sustained in its programs and activities. The cost of such medical expense would make program fees prohibitive. Therefore, each person registering him/herself for a program or activity should review his or her own health insurance policy for coverage. It must be noted that the absence of health insurance coverage does not make The Lyric Theatre & Cultural Arts Center automatically responsible for the payment of medical expenses. Your cooperation is greatly appreciated.

Release of Liability & Permission to Secure Treatment I recognize and acknowledge that there are certain risk of physical injury to participants in the above program(s) and I agree to assume the full risk of any injuries, damages or loss regardless of severity which I may sustain as a result of participating in any and all activities connected with or associated with such program(s).

Photography/Video Release:
I give permission for my image to be used in documentation of the program, including but not limited to website albums, print or media advertisement, etc. I have read and fully understand the above Release Liability and Permission to Secure Treatment and Photography/Video Release.
