The purpose of this questionnaire is to learn from FCCA members about the events of September 11, 2001, from the members’ perspectives and to preserve these memories for future generations.
1.What is your first and last Name?
2.What is your Court or Judiciary Office?
3.What is your current position?
4.Were you in your current position on 9/11/2001?  
5.Where were you when you first heard about the terrorist attacks on September 11, 2001?
6.What are some of your recollections about what you were doing at the time and how you responded to the news?
7.To whom did you reach out to at first?
8.What can you remember happening as news unfolded about the ongoing attacks?
9.How were you directly impacted by the tragedy (e.g., did you personally know someone who died, did you personally know a first responder, did you witness any part of the attacks, were you stranded on travel status?)
10.If you were working for the judiciary at the time, how did the court keep staff informed? Did court operations change in response to the attacks?
11.What would you like to share with fellow FCCA members about those dark days and how you and your colleagues coped?
12.If you could speak to any of the 9/11 victims, first responders, or any of their family members right now, what would you want them to know?
