LIFE Activities and Topics Survey for Employees

This is the monthly LIFE quiz for January 2026. Each employee will earn 5 points for answering this survey. Please answer all questions in order to make improvements to the LIFE program and keep things interesting. At the end of the survey, please hit the 'SUBMIT' button.
