Student Nomination Form

Instructions for your nomination:

Each organisation can nominate up to two students for the award. Please fill out a separate nomination form for each student, and submit by Monday 2 September 2019. We encourage early submission of nominations. Feel free to attach any supporting documentation or website links if appropriate.

You will be able to complete the nomination form more than once if you want to nominate more than one student. You will also be able to edit your responses while completing the form, but you will not be able to alter them after submission.

By ticking this box, you confirm that the nominated student will be over age 18 as of 31 August 2019 and has agreed to be nominated by your organisation, and that you and the student have read and agree to the terms and conditions of the Awards, and the Judging Criteria, which can be found on the Community Colleges Australia Conference website.
