This is an opportunity to participate in a voluntary survey regarding Utah's opioid crisis. This survey is being conducted by the Utah Opioid Task Force, Opioid Settlement Advisory Committee (OSAC). This team was created in order to understand the community's perspective on how to best address the opioid crisis.
- It will take approximately 10 minutes to complete this survey.
- The survey asks questions about your connections to the opioid crisis and about your insights on how it should be be addressed.
- Your answers will be used to help identify strategies to reduce the harms caused by Utah’s opioid crisis.
- Your participation in the survey is voluntary and your responses are anonymous. Your name and other identifying information will not be collected.
