Survey to Identify Chlamydia and Gonorrhea Toolkit Needs Among Indian Health Service Provide

The purpose of this survey is to further understand the knowledge, attitudes and practices around chlamydia and gonorrhea prevention, diagnosis and treatment and to identify chlamydia and gonorrhea tool kit resource needs. Your answers will help shape the development of a provider tool kit for chlamydia and gonorrhea screening and treatment.

This provider toolkit is being designed as a comprehensive content and resource guide which will include templates and current best practices with regard to education, testing and treatment of chlamydia and gonorrhea.  This toolkit will be used by health care professionals to improve chlamydia and gonorrhea screening rates and to improve patient care.
