Slow Food Chicago Constituent Survey

Slow Food Chicago (SFC) has developed the following brief survey to understand how to best serve our constituents--members and partners. Please note that each question requires an answer. By understanding your needs, we hope to better engage with you and develop new collaborations that benefit the Chicago area food community. Thank you in advance for your help to improve our efforts.
