Letter of Support Request Form

The Alzheimer Society of BC and Yukon champions research focused on prevention, diagnosis, and care in dementia, including providing letters of support for researchers applying for grants and awards.

To request a letter, please complete the form below and email Dr. Sarah Wu, Strategic Lead, Research, to let us know you are seeking support.

Once your request has been submitted, you can expect a response within 2 business days.

5.Please provide a short summary of the request, including a brief overview of the study, project or proposal relevant to the letter of support.(Required.)
6.Please provide a draft letter of support (format: .doc or .docx) which should include:

• To whom the letter should be addressed (individual name or organization), and the mailing address.
• If applicable, the funding organization and the specific name of the grant/award.
• The name and contact information of the person submitting the request.
• The full title of the research study, project or proposal.
• One or two paragraphs explaining how the study, project or proposal will impact people affected by dementia or the public.(Required.)
