Shelter Guest Survey

Conducted by the Sonoma County Lived Experience Advisory and Planning Board

The Lived Experience Advisory and Planning Board is a group of leaders with lived homelessness experience focused on advocacy and improving homeless services. Sonoma County LEAP Board is conducting a survey to assess the conditions of our emergency shelters. The survey will focus on understanding and evaluating participants' experiences. Using the information gathered, we will help develop solutions to address the identified needs and concerns.

In this survey, we hope to gather insight into your experiences with the shelter intake process, your overall experience at the facility(s), and the resources you have been provided to improve your situation.

Your responses can be anonymous. If you are comfortable being contacted by the LEAP Board for follow-up questions, please include your name and contact information at the end of the survey. We appreciate your contributions and thank you!
