Food and internet access survey for Marion C. Early R-V students

Parents,

In the event of a long term (8 days or more) school shut down, education would continue through the use of technology and/or paper copy materials. We recognize some families may have limited access to technology devices and internet services. We also recognize that many families rely on school breakfast and lunch programs for meals.

To better assess our families access to technology and food needs, we ask that you complete this short survey ASAP.
1.Does your child have internet access at home? 
2.Does your child have access to a Chromebook or other computer device for accessing the internet?
3.If you answered "NO" to either of the above questions, please list the FIRST and LAST names of all students who do not have a device or internet access.  Please separate each student with a comma.
4.In the event of a long term school closure, we would work with our state department to provide meals to students.  Please list the FIRST and LAST names of all students who would need breakfast and lunch provided to them.  Please separate each student with a comma.
