Introduction, confidentiality and guidance
Introduction: HQIP is seeking feedback from stakeholders on how well we support the re-invigoration of clinical audit as a whole, and how much we help your work on audit in particular.
Confidentiality: This survey has been anonymised - personal contact details are not being recorded, and this includes respondents' IP addresses. Responses are automatically limited to one response per computer and email sender to further ensure the integrity of results.
Guidance on completing the survey: This form is grouped into the following sections:
Page 1. (this page, scroll down) About you / Overall feedback
Page 2. National Clinical Audit support (including the National Joint Registry)
Page 3. Quality Improvement & Development support (including local clinical audit)
Page 4. Communications support
Page 5. Other HQIP work
For many respondents it will be relevant to complete more than one of these areas: please do not restrict yourself to only one category. However, if only one section is appropriate, please restrict yourself to that area of our work, but please do comment on your overall perception of your work.
Once you have completed a section - or to ignore irrelevant sections and move to subsequent areas of the survey - scroll to the bottom of the page and click 'Next'.
Ratings scales of 1-5 are used throughout this survey. In all instances, 5 = the most positive response and 1 = the least positive.
Thank you for supporting us in enabling effective evaluation of our work.
