*Consent forms must be signed by a parent or guardian in order for youth to participate in the Path of Choices event. The form can be signed at the registration table at the event.
*You will receive a confirmation email with your designated time slot during the week of September 22, 2025.
*Refreshments will be provided for attendees upon completion of the Path of Choices activity.
