Each state affiliate is asked to complete the following State Affiliate Benchmarking Survey by June 9th. Any state affiliate leader can complete this on behalf of your state affiliate.

Please complete the questions to the best of your ability. Even if you don't have all the answers, we appreciate you taking the time to fill out what you can.
