About the Awards

Our awards are to recognise an individual, or team, who has demonstrated excellence when working to improve the health and wellbeing of local people.

Guidelines for Entry

The nominee should be dedicated to improving the health and wellbeing of local people and may demonstrate excellence in:

  • Promoting quality patient care
  • Innovation in care or management
  • Reducing health / access inequalities for patients
  • Working in the wider team (e.g. with hospitals, pharmacies, VCSE sector)
  • Supporting their Practice, PCN, and/or Federation

This is a category for any team, except for a whole practice (who can be nominated for Practice of the Year). This includes: Nursing Teams, Management Teams, H&SH Teams, PCN Teams.
