Incident Report - An anonymous way to inform administrative staff of any incidents you wish to bring to attention.

You do not need to tell us any personal information about yourself in order to submit a form. If you would like to write your name you may if you wish. This form is for anything you wish to inform administrative staff at El Tejon Unified School District about such as safety concerns, bullying situations, anything at all.
