Welcome to the Western Districts Hockey Club Membership Survey 2019

Thank you completing this survey.  The information you provide will help us set the future direction of Western Districts Hockey Club (WDHC), and improve the quality of services we provide.  Your answers are anonymous, so please answer honestly. We only ask that you identify the team that you are associated with - the team with the most completed surveys will win a bar/kitchen tab to the value of $100.  Second and third placed teams will each win a $50 tab.

The survey will take about six minutes to complete.  For most questions, you simply need to indicate the extent to which you agree or disagree with a statement about the club by giving a star rating out of 10, with 10 indicating your strong agreement with the statement, and 1 indicating your strong disagreement with the statement.  Please select N/A if you don't know or if the question is not applicable to you.  There is a chance to add comments at the end of the survey.
