Play Like a Girl - Mentor Application

* Before applying, please agree to the following criteria:

• I understand that I will be a co-mentor to a small group of mentees.
• I will be able to meet my mentees, virtually, once a week (3-4 times per month) during the school day or after-school throughout the school year. 
• I do not foresee any moves or job changes that would prevent me from mentoring throughout the school year
• I understand that being matched with my mentees through her middle school graduation would be most ideal.
• I will attend an initial online orientation and new mentor training and participate in ongoing training, as needed.
