Viborg-Hurley School District 60-6

The Viborg-Hurley School District is preparing to open its doors on August 25 for on-campus instruction. We understand that some of our families may not be comfortable sending their children to school on August 25 for a variety of reasons. In an effort to plan for instruction for the 2020-2021 school year, please complete this survey for each of your children. DUE JULY 14 at NOON.

Question Title

* 1. Student's First and Last Name (You will need to complete one survey for each student)

Question Title

* 4. On-Campus Instructional Model:
Please note these items are subject to change or adaptation as new information evolves.

-          Older students and staff will be asked if they have been in contact with a person who has been diagnosed with COVID-19 and if they feel unwell with COVID-19 symptoms (cough, shortness of breath, breathing difficulties, sore throat). Younger students will be monitored for cough, flushed cheeks, rapid breathing, and fatigue. Staff and student temperature checks will occur when needed.
-          Parents will also be asked to check their child’s temperature before coming to school in the morning and to keep their child home if their child has a fever of 100° F or above, or if their child is feeling ill and has symptoms of COVID-19 (new cough, fever, breathing difficulties, headache, sore throat, runny nose).
-          Promote and encourage good health and personal hygiene
-          Provide additional hand washing opportunities
-          Increased cleaning and disinfecting of classrooms, supplies, and common areas.
-          Increase the distance between students as feasible.
-          No sharing of supplies OR disinfect between use.
-          Students must bring their own water bottle.

Distance Learning Model:

-          Expectations for distance learning will be significantly increased for the 2020-21 school year.
-          Daily participation and engagement in all courses.
-          Limited course offerings
-          Attendance will be taken daily
-          Expectation will be consistent with on-campus instruction and grading.

Given the highlights of the On-campus Instructional Model and the Distance Learning Model, what is your plan for the 2020-2021 school year?

Question Title

* 5. Hybrid Instructional Model:

There may come a point that internal health conditions or mandates by the State require the District to limit the number of students on-campus and we may need to consider a hybrid instructional model that consists of both on-campus learning and distance learning. This is not a desired model, but we must be prepared for all scenarios. We would make every effort to keep household members on the same schedule.

If a hybrid model of instruction became necessary, which option would you prefer?

Question Title

* 6. Transportation:
Please note these items are subject to change or adaptation as new information evolves.

Given the above information regarding transportation, please pick the option that best describes your plan for the 2020-2021 school year:
