The West Virginia Department of Education (WVDE) would like to get your feedback about your experience with Simulated Workplace. The Appalachia Regional Comprehensive Center (ARCC) at ICF is helping support WVDE in this effort. In order to help the WVDE determine if the Simulated Workplace program is meeting your needs, it is important to get your perspectives about the program.
We are asking that you complete this brief online survey. It should take only about 15 minutes to complete, and questions include a mix of items to rate your level of agreement, a few open-ended questions, and a few demographic items. These demographic items are for descriptive purposes only, and we are not asking for any personally identifiable information, such as your name, address, or any other sensitive information.
We will be taking care to protect your confidentiality for the answers you provide. Only WVDE and/or ARCC staff directly involved in this activity will have direct access to the database, and we will report findings only in an aggregated manner and/or by various subgroups (such as males vs. females, by counties, etc.). Since we are not collecting any personally identifiable information, your anonymity is ensured.
This activity presents only minimal risk, no more than a typical activity in a normal day. And, you are not required to answer any question that you do not wish to answer. Although we hope you will take the time to complete the full survey, you may stop participation at any time without penalty. Further, while there are no direct benefits or incentives for completing this survey, your feedback will help the WVDE better understand how participants are experiencing the Simulated Workplace program, which could lead to improved services in the future.
