INSTRUCTIONS for SCRAM SYSTEMS SERVICE PROVIDER/SUPERVISORY AGENT:
As the client completes the SCRAM CAM program and you perform the de-installation, please administer this brief two-part exit survey as follows:
- PART ONE - BASIC INFORMATION (2 minutes): The Service Provider/Agent answers questions 1-5 directly.
- PART TWO - THE SCRAM CAM EXPERIENCE (6-8 minutes): The Service Provider/Agent discusses questions 6-15 with the client and enters the client's responses before submitting the completed survey.
