The 2025 Illinois DeafBlind Project Service Provider/Professional Needs Assessment can be completed by individuals or on behalf of agencies. Responses are used to support immediate needs and set future priorities. It should take about five minutes, depending on your level of detail. Once a year a $50 Amazon gift card is raffled to all for responded that year who indicate in Question 8 that they want to enter the raffle. This year the raffle will occur July 1.
