The purpose of this report is to document, evaluate, and track chapter programming activities and event engagement. To stay on top of things, it is best practice to complete the report within one week of an event occurring.
Deadlines & Submission:
All activities must be submitted by the 5th day of the month following the event. The submission link will automatically disable on the 5th of each month and reopen on the 8th (e.g., September activities are due by October 5th; October submissions open October 8th).
Each Activity Report submission allows you to report up to 5 chapter programs. If your chapter completed more than 5 programs during the month, please submit an additional Activity Report.
