Annual General Meeting Registration Form

We, the HPA Board of Directors and the HPA Physician Representatives to the CCH-OHT extend a warm invitation to you to attend our ‘Annual General Meeting’ in person from 7:00 to 9:00 p.m. on March 20th. This event will occur in the Sixteen Mile room at the Oakville Hospital.
Please come and enjoy our inspirational guest speaker, Melissa Farrell, the President and CEO of Halton Healthcare. We’ve planned our AGM as an opportunity for you to discuss ways you want healthcare transformed to improve your lives as physicians. Through small group discussions, we intend to jump-start conversations on the future of healthcare in Halton. We will also provide a dessert and health snack table for all to enjoy.
