NOMINATION FORM FOR MEMBERS CHOICE CLUB OF THE YEAR AWARD
In 50 words or less, tell us why your club should win the Members Choice Award. There are prizes for the winning entrant and their club, so send in your entry today. Remember, the most creative entry wins. Good luck!
Terms & Conditions
1. Club members must explain (in 50 words or less) why they believe your club deserves to win.
2. The most creative entry will win a prize.
3. The winning nominated club will receive the ‘Members Choice Club of the Year’ award.
4. Examples could include: How the club serves its members, an occasion the club has helped a particular member of the community, or a specific fundraising achievement that was held in the interest of the club’s members or community.
5. Nominations close 5pm on 16th May 2016.
6. Submissions must be from a member of the nominated club and each club member can only send in one nomination.
7. The winning entry will be deemed the most ‘unique ’(original and creative) answer by the Clubs WA judging team
