Field Resource Navigator 

Pay Rate:  $19-$25

Part Time and Full Time

Position is Temporary with possibility of extension

 

Purpose of Position:

Under the supervision of a Resource Navigator Supervisor, performs both remote and field work in support of COVID-19 contact tracing, with alternative work schedules and/or compressed work weeks as necessary. Conducts interviews of individuals diagnosed with COVID-19 (“Cases”) and the individuals that they came into contact with (“Contacts”) about resource needs using a trauma-informed, culturally respectful approach that builds trust and facilitates the free sharing of information. Informs visitors to field sites about the available resources for potential Cases and Contacts in the event those visitors may need to isolate at home. Connects Cases and Contacts with identified resources.

 

Examples of Typical Tasks:

1.   Conducts telephone calls with persons diagnosed with COVID-19 and those identified as close contacts to assess resource needs for self-isolation and quarantine.

2.   Conducts in-person conferences with Cases, Contacts, and visitors to field sites established by the New York City Test & Trace Corps anywhere in the five boroughs of New York City to both assess resource needs for self-isolation and quarantine and proactively inform individuals being tested of the resources available to them if required to isolate at home.

3.   Identifies common needs amongst Cases and Contacts and connects them to local solutions. This could include access to food, mental health services, and other community programs helping patients in need and/or people in isolation or quarantine.

4.   Connects client with needed resources through various platforms.

5.   Communicates with Cases, Contacts, and visitors to field sites in a professional and empathetic manner.

6.   Collects and records information obtained from the resource interview and gathering into the data systems in accordance with standard operating procedures.

7.   Follows approved scripts and protocols, provides cases and contacts with vetted information about local community resources.

 

Qualification Requirements:

·         Two years of coursework or equivalent experience in social or human services or related fields (social work, human services, community/public health, sociology, psychology, mental health counseling, or related field)

·         Familiarity with and experience working in New York City health systems or social/human services

·         Ability to accept field postings anywhere within the five boroughs of New York City, including to sites that may not be easily accessible via public transit

·         Candidates preferred to have experience working with community resources and social services in different parts of New York City that can inform connecting patients with COVID and their families to local resources. (Please indicate what location on your application)
