Welcome to our survey! We appreciate your interest in understanding the risk factors that may affect your child's health. Our survey is a part of the Bright Futures program, a national initiative that provides guidance on how to take care of children and teens. We cover a wide range of topics that include physical health, growth, behavior, and mental well-being.
 
After you complete the survey, a school health worker will analyze the results and let you know if there are any areas of concern. The results will be mailed to you along with educational information to help support your child. If you have any questions or concerns, our school health providers will be available to review the results and provide assistance.
 
As the American Academy of Pediatrics states, "checking in and watching out for children's health is important for making sure they stay healthy and catch problems early" (American Academy of Pediatrics, 2018). By taking the Bright Futures survey, we can ensure that our children receive the best care possible and address any problems early on.
 
Please note that if your child is 0 to 15 years old, a parent must take the survey on their behalf. If your child is 16 years or older, either the parent or the child may take the survey.
