* 1. Dear Participant:

I am currently conducting undergraduate research while attending Illinois College with the intent of later publication. My research involves the analysis of emotions, developmental characteristics, and gender identity within the population.

I would like your permission to participate in this study by responding to a survey containing a series of measures.  Your permission allows me to understand your views, ideas, and perspectives on this topic.

Please be advised that all information gathered from this survey will be kept completely confidential.  In addition, information from this survey that I include in any written work will be discussed in such a way that no individual can be identified.

Also, please note that your participation in this study is completely voluntary, and that you may choose not to complete the survey once you have begun.

STATEMENT OF INFORMED CONSENT

I give my consent to participate in a survey on gender, emotions, and developmental characteristics for Natasha Polak, a student currently enrolled at Illinois College.

I understand that my participation in this survey is completely voluntary, and also understand that I may withdraw my consent at any time during or after the survey.

I have read the above and understand it and hereby consent to the procedures(s) set forth.
Risks and Benefits regarding this study:

 Possible negative impacts from this study would be the risk of depressive thoughts due to any perceived negative beliefs regarding results from the listed surveys. Benefits from this research will result in greater knowledge regarding Transgender Individuals, specifically in regards to treatment of depressive behaviors.
