Informed Consent

Question Title

* 1. Informed Consent.
You are being invited to participate in this study because you are an occupational therapist currently licensed and practicing in Nebraska. If this is not true, please do not complete the survey.

The purpose of this study is to examine the role of occupational therapists in Nebraska in the battle against childhood obesity. All information will be collected through an electronic survey, which consists of 15 questions and should only take 10-15 minutes to complete. The survey will not collect any personal information or any information where your identity could be revealed. Survey responses will not be linked to e-mail addresses. All questions focus on current occupational therapy practices.  Even though there are no direct benefits to you individually, results of this survey could benefit the profession. It could bring awareness of the childhood obesity problem to occupational therapists and hopefully influence them to take action.

Furthermore, you may leave survey questions unanswered, and you are free to withdraw consent and to discontinue participation at any time without prejudice to your future relations with Wayne State College, their professors, or the principal investigator.
