Overview
In preparation for the completion of the Congressional Gold Medal by the US Mint, scheduled ceremonies and events, we request that eligible applicants provide us further details to assist in the planning. Questions can be sent to CGMCommittee@caww2.org 

Step 1
Prior to completing this form, please verify that the Veteran's name appears on the list of approved CAWW2 Veterans. If the Veteran's name does not appear on the list, ensure that an application was submitted directly to the CAWW2 website and not through other means. If you submitted an application within the last 35 days, please wait until the Committee has an opportunity to review the application prior to contacting us. If the Veteran's name appears on approved on the website, please proceed to step 2.

Step 2
A single replica Congressional Gold Medal will be presented to each Veteran at no charge. If the Veteran is no longer living, the CAWW2 Recognition Project will present a single Medal to the Next of Kin at no charge. Friends are not eligible to receive the Medal. In some instances a Veteran may have more than one approval letter from the Committee due to applications submitted by siblings, relatives, and friends. Regardless of the number of letters, only a single no-charge Medal will be presented.

All Award of Medal forms must include a Next of Kin. Please coordinate internally with your family on who will be the designated Next of Kin. The definition of Next of Kin can be found here. Duplicate Award of Medal forms submitted from multiple family members will delay the final award of the medal.

The Next of Kin assumes the role as the designated Best Point of Contact and will be responsible to the Veteran and their immediate family for:
  • Notifying them of the Veteran’s receipt of the CGM
  • Notifying them of updates and progress for activities associated with the Recognition Project activities, including scheduled ceremonies.
  • For the Next of Kin, receiving the Congressional Gold Medal on behalf of the Veteran in their absence.
  • Acting as the Responsible Party for the Veteran for their care and welfare during their participation at all ceremonies attended.
