Survey for Residential Aged Care Homes (RACH)
As your local Primary Health Network, Eastern Melbourne PHN (EMPHN) is focused on enhancing care for older individuals in Residential Aged Care Homes (RACHs). EMPHN has received Department of Health and Aged Care (DoHAC) funding to assist with the implementation of two programs relevant to your RACH:
The General Practice in Aged Care Incentive (GPACI) and
To assist us in planning activities to support GPACI and MHR implementation, we are gathering information on RACHs and the GPs who are currently providing services to their residents.
The survey takes about 12 minutes to complete, and only one survey per RACH is needed.
