Program Description for Participants and Families

We are excited to offer the Life After High School program in partnership with REAL Mentoring, funded by the Governor's Prevention Partnership. This unique opportunity is available to youth ages 12-17 from Hartford and Torrington, offering 15 slots with monthly mentoring sessions (4 hours per
month). The program is designed to provide personalized guidance and support, helping young individuals develop life skills and navigate their future with confidence.

During these 4 hours each month, participants will engage in a variety of activities, including:

Workshops and Group Discussions: Focused on career exploration, education pathways, life
skills, and setting future goals.
Case Management and Support: Offering individualized attention to help youth overcome obstacles and find solutions for personal growth.
Prosocial Activities: Encouraging positive social interactions that build strong relationships and community engagement.
Exploration Learning and Fun Activities: Creating opportunities for hands-on experiences and adventures that foster curiosity and new interests.
Mentoring Sessions: Offering guidance to strengthen decision-making, confidence, and life direction, all while reinforcing positive behaviors and reducing the likelihood of substance use.
