Introduction

This survey should take about 10-15 mins and is intended to be completed by people with disability or by their family member or carer on their behalf. Regardless of who is completing the survey, the questions should be answered in relation to the person with disability.

Please note, this survey is only intended for people with disability living in the Northeast Victorian region (Alpine Shire, City of Wodonga, Indigo Shire, Rural City of Wangaratta and Towong Shire) as we operate and deliver programs & services only in these locations.

The insights gained will be used to guide future program delivery and to tailor potential sports club/association workshops in this space.
