Chapter of the Year Nomination Form

Identify activities over the chapter’s lifetime with CHADD, including any specific outstanding leadership qualities over the past year that demonstrate the nominee’s contributions to CHADD’s mission, including but not limited to providing information and support to the community, orienting volunteers, outreach to the community and diverse populations, and developing relationships with other organizations.
The Volunteer of the Year does not have to be associated with your Chapter of the Year nominee. This person can be from a different chapter or satellite. However, you can also nominate a Volunteer of the Year who is associated with your Chapter of the Year nominee.
