NACWA is conducting this survey to collect information about technical, legal, and policy issues related to the practice of blending.  For the purposes of this survey, blending is defined as routing primary treated effluent around secondary treatment (with or without parallel treatment) and recombined before discharge.  The survey results will be used in NACWA’s advocacy work for a consistent, national blending policy.

Utilities are asked to complete this survey by September 4, 2015.  The survey must be completed separately for each treatment plant operated by a clean water agency.  Partially completed surveys cannot be saved.  The entire survey in PDF form is available at www.nacwa.org/blendingsurvey.  Since survey respondents will be directed to different pages of the survey depending on their answers to certain questions, respondents may not see all of the questions that appear in the PDF version when completing the actual survey.  The question numbers in the actual survey and the PDF may therefore also not match.  

The information generated from this survey is confidential research information being collected by NACWA.  The utility name and contact information will be kept confidential and will only be used by NACWA for quality control purposes.  This information will not be reported to EPA or any other entity.  Other information submitted for the survey will only be used in anonymous, aggregate form.
