BackPack Parent Survey 2016-2017

We want to thank you and your family for being a part of the Brazos Valley Food Bank's BackPack Program. We ask that you please take a few minutes to compete this evaluation. Please give as much information as possible as this will help us understand how the BackPack program works for your child.

If you fill out the paper copy of this survey, have your child turn in the completed survey to their BackPack coordinator.
