Dear Student

We are interested in your thoughts and views about various situations you may encounter at school or in your community. These situations focus on issues that can happen between individuals or between individuals in some type of relationship. We are only asking about your opinions and what you believe would be your response to some of the situations.

Your opinion and thoughts on these questions are very important to us. Every student's response is important.

There are no wrong or right answers. We are only asking about your opinions and reflections about issues. Your responses will be anonymous--there will be no way to connect you to the answers you provide.

Your participation in the survey is voluntary. You may skip any question you are uncomfortable in responding.

On occasion it may be helpful to come back to students to ask some of the questions again later in the school year.  This helps us understand how your views may, or may not have changed, as time goes on.  It is important to be able to link the answers you give today, with answers you may give at another point. We want to be able to do this so your answers are anonymous. In order to do this without using your name, we will need to (re)create a unique code for you.

The school counselor or Title IX Coordinator may be a resource for you should any questions raise an issue for you that you would like to discuss.
