2026 ESAC Membership Application Package

This application form is intended for applicants to the CLHA Education Standards Advisory Committee (ESAC).

Please ensure that all components of the application package are complete before submitting.

Applications will close on September 28, 2026, and any incomplete forms will not be saved.

2.Applicants will be assessed based on the following qualifications and experience. Applicants are not required to meet every item.

3.Are you free from any actual or perceived conflicts of interest, including having served on Council or a CLHA committee within the past two years or being a current CLHA staff member?(Required.)
4.A computer is provided to ESAC members to facilitate their work. Are you reasonably proficient with MS Office, downloading and uploading documents, and feel comfortable using various technologies for virtual meetings?(Required.)
5.ESAC members participate in virtual meetings and online group discussions. Are you comfortable actively participating, sharing your perspectives, and collaborating with others in an online environment?(Required.)
6.Applicants must submit a completed Applicant Statement and Information Form. Please download, complete, and upload the Applicant Statement and Information Form.(Required.)

7.Applicants must be prepared to provide a written reference from a current or previous supervisor. Please download, complete, and upload the ESAC Application Reference.(Required.)

All applicants will be reviewed and considered by the Appointment Committee, should your application be selected to move forward to the next phase of this process you will be contacted to meet for a formalized discussion.
