Title of the Project: A Survey for Christian Women

Invitation to be part of a Research Project:
You are invited to participate in a research project. To participate, you must be over the age of 18, identify as female gender, proficient in English, and identify as a Christian believer. Taking part in this research project is voluntary through your agreement with Set Apart Pastoral Counseling & Coaching LLC, and Dr. Kate Bohannon.

What is the project about and why is it being done?
The purpose of the project is to gather information about the lives of Christian women through their personal experiences.
What will happen if you take part in this project?
If you agree to be in this project, we would ask you to do the following:
1. Complete the Online Survey. The completion time is between 3 to 15 minutes, depending on the survey link selected.
2. Consider sharing the survey link with a friend or family member to help us reach our participant goal.
How could you or others benefit from this project?
Participants should not expect to receive a direct benefit from taking part in this survey.
Participants will not be compensated in any form for the completion of their surveys.
Benefits to society include essential information for counselors and church laypersons for future therapeutic services.
This project will directly benefit the future decision-making of Dr. Kate Bohannon and Set Apart Pastoral Counseling and Coaching, LLC.
What risks might you experience from being in this project?
The risks involved in this study are minimal. Minimal risks are equal to the those you would encounter in everyday life.
How will personal information be protected?
The required survey questions are confidential and anonymous. Your personal information (name, address, email) is not required.
Participant responses will be kept anonymous. Participant responses will be reported anonymously.
The digital record of this project will be kept privately on a password-locked computer. Data will be stored on a password-locked computer and may be used in future presentations. Research records will be stored securely, and only the researcher will have access to the records. After five years, all electronic records will be deleted, approximately July 2027.
However, if the survey participants select at the end to be included in future correspondence by Dr. Kate Bohannon or Set Apart Pastoral Counseling and Coaching, LLC about additional surveys, space for the participant to leave an email address is encouraged.
Is survey participation voluntary?
Participation in this survey is voluntary. Your decision about whether to participate will not affect your current or future relations with Dr. Kate Bohannon or WOWSingleMOM ministries. If you decide to participate, you are free to skip any of the non-required questions, and you may withdraw at any time before the survey data is collected.
Likewise, participation in this survey does not constitute a therapeutic relationship between the participant and Dr. Kate Bohannon.
