Thank you for participating in our survey. Your feedback is important.  You must be 18 years old and Oregon resident to complete this survey.  

Chinese Friendship Association of Portland http://www.portlandcfa.org/ is working on a project to identify current gaps in housing and facility-based residential service for people with behavioral health needs, so that we can make recommendations to Oregon Health Authority and other agencies on how to best invest corresponding funding.   The survey will be anonymous and confidential.  Through this survey, we hope to obtain community data for analysis and propose for our recommendation.
