Outstanding Partner Recognition Nomination Form

We are looking for nominations for partners who have helped build the college over the last 20 years.

Two external partners (individuals or organizations) will be selected (one in 2026; one in 2027) to receive a commemorative recognition plaque and College of Public Health swag package in appreciation of their impact and partnership.

The Outstanding Partner Recognition Award honors external partners who have made exceptional contributions to the College of Public Health through collaboration, leadership, service, advocacy, innovation, or sustained commitment to advancing public health.

Nominees may be individuals or organizations that have worked with the College through education, research, practice, service, workforce development, community engagement, or other collaborative efforts.
