Dear prospective YAB member--thank you for your interest in being a part of our board for the 2021-2022 school year. Being a part of YAB is a big commitment, but also a wonderful opportunity for growth, leadership and service.

After receiving your application, myHealth staff will review applications and select candidates for interviews (held summer of 2021). Not all candidates who apply will receive interviews, and not all candidates who are interviewed will be offered a position on the board.

Here are the expectations of any potential members. If you do not feel that you can meet all of these expectations, please do not complete the application.

As a member of the Youth Advisory Board you will:
-Have a significant board member position to list on your college applications
-Be eligible to receive a signed letter of recommendation for you to use as you need *
-Become educated on health topics and gain opportunities to utilize that knowledge to help your peers
-Have the opportunity to apply for chair positions
-Develop leadership and communication skills
-Meet young people from other high schools

myHealth is committed to:
-Lead meetings in ways that are creative, fun and accurate.
-Treat you with respect and honor your time and contribution.
-Arrive at each meeting early and prepared with worthwhile, meaningful activities.
-Answer questions honestly, openly and in a non-judgmental way.
-Support you in prioritizing school, family and self-care commitments.
-Provide you with safe and challenging opportunities for growth.
-Ask for and listen to your input on the program.
-Make sure you have a GREAT time as a YAB member!

Expectations for you:

Participation and Attendance
-Attend all scheduled bi-monthly meetings, events and service learning projects. If you cannot attend, you must submit for an excused absence within 48 hours of the scheduled meeting, event or service learning project. No more than three (3) unexcused absences are allowed during the year. Reply to all texts from myHealth staff in a timely manner so they know you’ve received the message
-Represent myHealth in the community by spending at least 4 hours per year at tabling events
-Commit to assisting with preparation, set up, clean up or work during or before the myHealth Gala (held in early October)
-As a group, volunteer at least 3-4 times per year in the community to represent myHealth

Marketing and Review
-Review myHealth materials or come to the clinic locations and give your opinion on décor, exam room set up, staff friendliness, ease of accessing services, etc. This may include being a “secret shopper” to provide feedback about our clinic services
-Be the face of myHealth at your school by representing the programs and services we offer for young people ages 12-26 and refer others to us whenever possible
-Interact positively and enthusiastically with community members while tabling, volunteering or doing peer education
-Have your photograph, FIRST name and story used in media related to myHealth (though if you are a client, this information will not be revealed to anyone, including family members (per the MN minor consent law), only that you are a member of the Youth Advisory Board. If you are under 18, this will require a photo-release SIGNED by your parents

Events and Fundraising
-Help plan two (2) Youth Advisory Board fundraisers for myHealth and promote them to your family, friends and teachers
