Dear Participant,

This letter is to introduce Terenzio Gaetan and Miranda Green who are Honours students in the School of Health Sciences at Flinders University. They are undertaking research leading to the production of a thesis or other publications on the subject of exercise, nutrition and well-being.

I would like to invite you to assist with this project. No more than 20-30 minutes of your time on one occasion will be required to take part in the study. Participation is voluntary. If you are interested in participating in this study, please read through the information sheet and then if you wish, fill out the survey by clicking next on the screen below. Upon completion, your data will be entered into a secure, password protected server that can only be accessed by the researchers. By beginning the survey, you are agreeing to participate in the study and providing the researcher your informed consent for your participation.

Be assured that any information provided will be treated in the strictest confidence and none of the participants will be individually identifiable in the resulting thesis, report or other publications. You are, of course, entirely free to discontinue your participation at any time or to decline to answer particular questions. Although we think it is unlikely, should any of the items within the questionnaire raise any issues for you, please contact Lifeline on 13 11 14 or your family GP.
